Information below must be read and signed/initialed in order to take advantage of the services provided by O2-Wellness. Separate forms will be provided for facial Treatments. If any of the following conditions exist, we strongly advise consulting your physician prior to using our services.

Please be aware of the following:

- Do not wear metal, operate a computer or use a cellular phone during an ionSpa Pro session.
- Consult your physician prior to your ionSpa Pro session if you have any of the following conditions: diabetes, implants (including dental), congestive heart failure, or any other medical condition.
- You must be properly hydrated prior to each ionSpa Pro session.

By signing below, I have read and understand all of the contraindications for the Therapeutic Whole Body Vibration Machine, Infrared Sauna, Ionic Foot Detox, Lipo Light, Ultrasonic Cavitation/RF, Oxygen Bar, and Body Balancing Therapy.

I am using the Therapeutic Whole Body Vibration Machine, Infrared Sauna, Ionic Foot Detox, 10 Minute Face Lift, Oxygen Bar, Body Balancing Therapy, and the HCG diet drops and plan at my own risk and I understand it is my responsibility to inform O2-Wellness of any pre-existing medical condition. O2-Wellness will not be held responsible for causing or exacerbating any personal injury.

I also acknowledge that the information and products provided by O2 Wellness website, informational packets, or employees is not a substitute for professional medical diagnosis or treatment by a licensed physician. Information and statements regarding our products are for informational purposes and have not been evaluated by the Food and Drug Administration (FDA) and are not intended to diagnose, treat, cure, or prevent any disease. I also understand that O2 Wellness encourages me to always seek the advice of my physician or other qualified health provider with any questions I may have regarding a medical condition.

I hereby certify that I am in good physical condition and I am not aware of any physical infirmity that presents a risk with the services provided by O2 Wellness. None of the contraindications on this sheet apply to me. Should there be a change in my medical condition after signing this form, I agree to inform O2 Wellness and review the contraindications prior to using the aforementioned services.

I am fully aware of the risks and hazards connected with the services provided by O2 Wellness. I agree not to sue or bring any legal action against O2 Wellness, their employees, owners and agents, and I release all aforementioned parties from any and all liability claims, liens, or causes of action resulting from any damage, loss or injury (including death) while using their services, regardless of the cause of such loss, injury or damage. I acknowledge and understand that this is my responsibility to Inform O2 Wellness to any pre-existing medical condition or injury. O2 Wellness will not be held responsible for the causing or exacerbating of any personal injury, and are therefore held harmless.
